Homeless Outreach Coordinator (two positions)
Blue Ridge Area Coalition for the Homeless (BRACH)
Funding Period: July 1, 2026 – June 30, 2027
The Blue Ridge Area Coalition for the Homeless (BRACH) is seeking proposals from qualified organizations to provide Homeless Outreach Coordinator services for the VA-504 region. BRACH is the Continuum of Care Lead Agency for the Greater Charlottesville area and is a community‑based homeless services non-profit dedicated to homelessness prevention, emergency shelter coordination, transitional housing pathways, permanent supportive housing, and data‑driven community collaboration to end homelessness across the region.
Project Overview
The Homeless Outreach Coordinator will support outreach efforts throughout the community, strengthen connections to housing stability resources, and participate in regional efforts aimed at reducing chronic homelessness. This role will focus on engaging unsheltered individuals, coordinating with local partners, and connecting people to shelter, rapid rehousing programs, and supportive services.
Scope of Work
Proposals should outline the applicant’s experience and approach related to:

Conducting outreach to individuals experiencing street homelessness

Coordinating with local agencies, emergency shelters, and community partners

Facilitating access to affordable housing and low‑income housing support resources

Supporting data collection for regional homelessness data‑driven solutions
-
Providing documentation and communication for case management activities

Ensuring culturally competent and trauma‑informed service delivery

Outreach case conferencing:

How outreach case conferencing meetings will be organized and facilitated

Communication methods between outreach teams and partner agencies
-
Processes for follow‑ups, shared case responsibilities, and coordinated care

Funding
Funding for two Homeless Outreach Coordinator positions is provided by the Blue Ridge Area Coalition for the Homeless for a one‑year term beginning July 1, 2026 and ending June 30, 2027.

HUD Continuum of Care Program
The Continuum of Care (CoC) Program is designed to promote communitywide commitment to the goal of ending homelessness by providing funding for efforts by non-profit providers and state and local governments and promoting access to and effect utilization of mainstream programs by homeless individuals and families.
BRACH serves as the Collaborative Applicant, as designated by the Continuum of Care.
The Notice of Funding Opportunity (NOFO) for the Fiscal Year (FY) 2026 Continuum of Care (CoC) Program Competition has been posted at this link.
The CoC Application, CoC Priority Listing, and Project Applications will be available in e-SNAPS. Collaborative Applicants and project applicants will be able to access the applications to review, update, and enter information that is required for the application process.

Project proposals are due to the CoC Lead Agency (BRACH) in e-SNAPS by July 31, 2026.
The BRACH Governance Board’s Project Evaluation Committee will score and rank projects using the 2026 rating and review procedure, which will be posted when the application is live.
Homeless and Special Needs Housing Grant Programs
The mission of the Homeless and Special Needs Housing unit (HSNH) within the Virginia Department of Housing and Community Development (DHCD) is to ensure homelessness is rare, brief, and non-recurring. To that end, HSNH administers a continuum of state and federally-funded homeless service programs to address housing and stabilization services for individuals and families at-risk of or experiencing homelessness in the Commonwealth. HSNH works closely with communities and an array of service providers including non-profit organizations, units of local government, and housing authorities, to ensure comprehensive homeless services are provided effectively and efficiently in accordance with best-practice models to maximize limited resources.
